Job Summary
The ACT Case Manager at PermiaCare assists individuals with severe or chronic mental illness by providing treatment, rehabilitation, and support services. This role involves coordinating and delivering services to help clients maintain stability, prevent hospitalization, and support their overall mental health.
Responsibilities
- Provide rehab services to clients in various settings.
- Assist with medication management and administration.
- Offer transportation to clients as needed.
- Ensure accurate and timely documentation of services provided.
- Collaborate with team members and other professionals to support clients' needs.
Requirements
- Bachelor's degree in a related field.
- At least 1 year of experience in the mental health field preferred.
- Valid Texas Driver's license and auto insurance.
- QMHP certification required within 6 months of hire.
