Advanced Practice Registered Nurse, Programs of All-Inclusive Care for the Elderly (PACE)

Job Description

Esperanza Health Centers is looking for an Advanced Practice Registered Nurse with a focus on Programs of All-Inclusive Care for the Elderly (PACE). In this role, you will provide primary medical care services to older adult participants, emphasizing prevention and healthy lifestyles within the PACE model.

Responsibilities

• Manage primary care medical practice in PACE Center and participants' homes.
• Promote patient satisfaction and acknowledge factors influencing health.
• Participate in Quality Improvement activities and peer reviews.
• Practice evidence-based primary care in a team approach model.
• Maintain clinical competency through ongoing education and training.
• Collaborate with partner hospitals and healthcare professionals.

Requirements

• Participate in community outreach activities as per Esperanza's mission.
• Bilingual in English and Spanish preferred.
• Certified in CPR/BLS and possess a valid driver's license and automobile insurance.
• Graduate of an accredited nursing program with relevant experience.
• Current IL State License and Board certification in Primary Care.
