Position Summary

Comhar Inc. is seeking a Bilingual Blended Targeted Case Manager to join our community-based healthcare organization. The role involves providing blended case management services to adults with mental health needs, focusing on the Hispanic community and young adult population. As a Case Manager, you will work directly with individuals to assess their needs, develop service plans, and connect them with essential resources for improved well-being.

Responsibilities

• Assess clients' needs and develop comprehensive service plans.
• Link clients to community resources and monitor their progress.
• Maintain accurate client records and submit necessary documentation.
• Provide crisis intervention and support for individuals in need.
• Coordinate treatment programs and ensure client adherence.
• Meet productivity requirements and promote community-based services.

Requirements

• Fluency in English and Spanish preferred.
• BA degree with 2 years of case management experience.
• Valid driver's license and team-oriented mindset.
• Strong communication, flexibility, and computer skills.
