Blended Case Manager Supervisor

Job Description

Comhar Inc. is seeking a dedicated Blended Case Manager Supervisor to join our dynamic team. As a Blended Case Manager Supervisor, you will be responsible for overseeing and providing on-site supervision to Teams of Blended Case Managers, ensuring the highest quality of care for our participants.

Responsibilities

• Assure adherence to DBH Practice Guidelines in daily operations.
• Provide daily on-site supervision to Teams of Blended Case Managers.
• Conduct weekly individual supervision sessions and maintain detailed documentation.
• Monitor staff productivity and documentation quality.
• Ensure compliance with Pay 4 Performance criteria.

Requirements

• Masters Level degree in social work, psychology, or related fields.
• Supervisory experience in mental health direct care.
• Knowledge of Blended Case Management Standards of Care.
• On-call responsibility and valid driver's license.
• Bilingual (Spanish/English) preferred.
