Kolmac Outpatient Recovery Centers is a leading provider of outpatient treatment in the Mid-Atlantic region. We are looking for a dedicated and experienced Care Coordinator to join our team. As a Care Coordinator, you will be responsible for engaging with outside entities, building referral relationships, assessing patient needs, and providing support throughout their treatment.

Responsibilities

• Perform care coordinator duties within designated scope and ethical guidelines.
• Maintain a patient caseload and provide weekly coordination of care.
• Assess patient needs, create discharge plans, and motivate continued care.
• Collaborate with the interdisciplinary team to ensure quality of care.
• Adhere to HIPAA and 42 CFR Part 2 guidelines.

Qualifications

• High school diploma required; bachelor's degree preferred.
• Valid driver's license.
• 2 years experience in behavioral health or addiction treatment.
• Experience in the recovery community or addiction treatment field preferred.

Expected Travel: Travel required, gas reimbursement provided.
