Job Description

We are seeking a dedicated Care Manager to join our team at the Agency on Aging of South-Central Connecticut. As a Care Manager, you will play a crucial role in helping older adults, people with disabilities, and their caregivers access the support and services they need to age in place and stay connected to their communities.

Responsibilities

• Conduct comprehensive assessments of clients' health, functional, and cognitive status.
• Educate clients on available resources and program services.
• Assist clients in completing applications for supportive services and programs.
• Monitor and adjust care plans as needed in collaboration with clients and their support systems.
• Ensure timely and accurate documentation and reporting.
• Meet critical deadlines to expedite access to essential services.

Requirements

• Bachelor's degree in health, social work, gerontology, nursing, or related field.
• Minimum of one year of experience working with older adults or individuals with disabilities.
• Excellent interpersonal and organizational skills.
• Familiarity with community resources and state-funded program documentation requirements.
• Bilingual proficiency is a plus.
