As a Care Manager, you will provide quality care and services to residents in our assisted living and reminiscence neighborhoods. Responsibilities

• Provide assistance with activities of daily living and attend to individual care needs.
• Develop Individualized Service Plans and participate in monthly updates.
• Ensure the well-being and safety of residents, notifying management of any changes in condition.
• Engage residents in enriching activities and maintain a supportive community environment.

Qualifications

• Prior experience working with seniors is preferred.
