Case Manager

Job Description

Santa Fe Recovery Center is looking for a dedicated Case Manager to join our Women and Children's Residential Program. The Case Manager will work closely with clients, the interdisciplinary team, and external resources to ensure successful treatment outcomes and care continuity. This position plays a crucial role in empowering mothers in recovery and breaking the cycle of addiction within families.

Responsibilities:

• Assess clients' needs and establish goals for treatment.
• Coordinate with community services and support networks.
• Provide education on behavioral health and life skills training.
• Maintain client confidentiality and accurate documentation.

Requirements:

• Excellent communication and interpersonal skills.
• Knowledge of HIPAA regulations and addiction-related behaviors.
• Ability to organize workload and manage diverse tasks.
• Case Management certification and relevant experience preferred.
