Case Manager - Community Support Program (CSP)

Job Description

Wisconsin Community Services is looking for a dedicated and compassionate Case Manager to join our Community Support Program (CSP). In this role, you will provide crucial support and services to clients with mental illness and substance use disorders. Responsibilities

• Perform case management in clients' homes and in the community.
• Support clients in daily activities and recovery efforts.
• Work as part of an Assertive Community Treatment (ACT) team.
• Facilitate group sessions and provide crisis intervention.
• Coordinate care with clients' natural supports and healthcare needs.

Requirements

• Good clinical skills and knowledge of mental health and substance abuse disorders.
• Effective communication and crisis response abilities.
• Strong organizational and computer skills.
• Bachelor's or Master's Degree in a Behavioral Science.
• Valid driver's license and automobile with insurance.
