Case Manager (RN)

Job Description

We are looking for a dedicated Case Manager (RN) to join our team at Molina Healthcare. In this role, you will be working closely with members, providers, and interdisciplinary teams to assess, plan, and coordinate the delivery of integrated care across various healthcare services. You will play a vital role in ensuring that patients receive quality, cost-effective care that aligns with their health needs and goals.

Responsibilities

• Complete comprehensive assessments of members and determine eligibility for case management.
• Develop and implement case management plans in collaboration with members and healthcare professionals.
• Conduct face-to-face or home visits as necessary.
• Monitor care plans, document interventions, and suggest adjustments for optimal outcomes.
• Provide ongoing management and support to assigned member caseload.
• Promote integrated services to enhance continuity of care for members.
• Facilitate interdisciplinary care team meetings and collaboration.
• Utilize motivational interviewing techniques and clinical guidelines to drive positive change in member behavior.
• Assist members in overcoming barriers to care through coordination and support.
• Travel locally as needed.
• Provide consultation and education to non-RN case managers.
• Handle cases involving complex medical conditions and medication regimens.
• Perform medication reconciliation when necessary.

Qualifications

• Graduate from an Accredited School of Nursing. Bachelor's Degree in Nursing preferred.
• 1-3 years of experience in case management, disease management, or related fields.
• Active, unrestricted State Registered Nursing (RN) license.
