Responsibilities

• Deliver skilled nursing care and treatment to geriatric patients in the community.
• Implement patient-centered care plans in alignment with available resources.
• Contribute to discharge planning for patients.

Requirements

• High School Diploma or GED.
• Current DE LPN license or compact state license.
