Job Description
Community Behavioral Health is looking for a Clinical Care Management Specialist to join our dedicated team. As a Clinical Care Manager, you will play a crucial role in providing intermediate or high-intensity care management to ensure holistic and effective care for our members. You will work directly with members, providers, and community partners to develop comprehensive care plans and monitor treatment quality. Additionally, you will facilitate care coordination, utilize evidence-based treatments, and maintain documentation to support quality care delivery. This hybrid role involves in-home visits and office-based work in the Philadelphia area.
Responsibilities
- Facilitate care coordination and Plan of Care meetings.
- Conduct Utilization Management activities and authorization of treatments.
- Collaborate with internal and external stakeholders for quality care delivery.
- Apply clinical knowledge to ensure timely access to behavioral health services.
Requirements
- Master's degree in Social Work or related field.
- Willingness to obtain licensure within the first two years of employment.
- Minimum 3 years of direct clinical care experience in behavioral health settings.
- Excellent communication, interpersonal, and time management skills.

Job Type: Full-time Salary: $65,600 per year
