Job Summary:

The CSC Case Manager at PermiaCare will be responsible for providing psychoeducation, psychosocial rehabilitation, and case management services to individuals enrolled in the CSC-FEP program. The role involves assisting individuals between 15 to 30 years old who are experiencing their first onset of a psychotic disorder. The Case Manager will work collaboratively with a multidisciplinary team to ensure individuals' mental health needs and recovery goals are addressed.

Responsibilities:

• Provide direct services and support to individuals in the CSC-FEP program.
• Develop and implement comprehensive recovery plans in collaboration with individuals and their families.
• Ensure timely and accurate documentation of services provided.
• Coordinate with internal and external stakeholders to meet individual's needs.

Requirements:

• Bachelor's degree in psychology, social work, or related fields.
• Minimum of 1 year experience in social services (preferred).
• Texas Driver's license, QMHP certification, and clear background checks.
• Knowledge of mental health treatment and HIPAA regulations.
