Responsibilities

• Manage a patient panel with diverse healthcare needs.
• Conduct acute, chronic, and well visits in-office and through tele-health.
• Collaborate with in-office and virtual clinical teams.
• Utilize clinical training to perform in-office procedures.

Requirements

• Completed an accredited FNP or PA program with national certification.
• 2 years of practice in outpatient primary care or equivalent.
• State licensed in New York by start date.
• Strong communication skills and the ability to provide care to individuals of all backgrounds.
• Proficiency in evidence-based primary care practices.
