Job Description

Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA, is currently looking for a Healthcare Admissions Coordinator to join our dedicated team. Responsibilities

  • Manage health services lead information and coordinate admissions to both Assisted Living and Healthcare.
  • Act as a liaison between Westminster Canterbury and community referral sources, calling on physicians, hospitals, skilled nursing facilities, and case managers to promote resident admissions.
  • Notify departments of new resident move-in dates and locations, and serve as a social worker for the skilled nursing/rehab residents on the rehabilitation unit.
  • Work closely with residents and their families during their rehab stay, meeting psycho-social needs, completing social services assessments, and contributing to treatment plans.

Requirements

  • Bachelor's Degree in a human services field is required.
  • Minimum of 5 years of healthcare experience is essential.
