Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. Our HomeHealth LPNs will provide skilled nursing care and treatment to geriatric patients in the community, implementing patient-centered care plans with dedication and professionalism.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
