Home Care LPN - Available for Per diem, Part Time, Full Time

Job Description

We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning. This role offers flexible scheduling options and competitive hourly pay rates based on experience.

Responsibilities

• Provide skilled nursing care and treatments to geriatric patients.
• Implement patient-centered care plans in collaboration with the healthcare team.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED.
• Current DE LPN license or compact state license.

Job Types and Compensation

We have opportunities for Per diem, Part-Time, and Full-Time LPNs with pay ranging from $22.00 to $28.00 per hour. The expected weekly hours vary from 20 to 40 hours per week.
