As a Home Care LPN at ChristianaCare, you will play a vital role in providing skilled nursing care to geriatric patients in the community. We are looking for experienced LPNs who are ready to excel in their career and support our patients with compassion and expertise.

Responsibilities

• Give skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement care plans that are goal-oriented, patient-centered, and resource-consistent.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED required.
• Must possess a current DE LPN license or compact state license.
