Job Description
Join our team at ChristianaCare as a Home Care LPN. Your role will involve implementing care plans, participating in discharge planning, and supporting patients with a goal-oriented approach. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High school diploma or GED required.
- Current DE LPN license or compact state license.
