Job Description
We are looking for an experienced Home Care LPN to join our team at ChristianaCare. As a member of our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement and monitor patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
