Job Description
ChristianaCare HomeHealth is looking for dedicated Home Care LPNs to join our team. As a Full Time, Part Time, or Per Diem LPN, you will provide skilled nursing care to geriatric patients in the community. Responsibilities
- Deliver skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
