Job Description
We are looking for experienced Home Health Licensed Practical Nurses (LPNs) to join the team at ChristianaCare. As a member of our dedicated staff, you will provide skilled nursing care and treatment to geriatric patients in the community, ensuring they receive quality care and support.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
