Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a member of ChristianaCare HomeHealth, you will provide skilled nursing care and treatment to geriatric patients in the community, implementing care plans that are goal-oriented and patient-centered. If you possess a current DE LPN license or compact state license and are dedicated to delivering exceptional care, we want to hear from you.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented and patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.
