Job Description
We are looking for an experienced Home Care LPN to join our ChristianaCare team. As a Home Care LPN, you will provide skilled nursing care to geriatric patients in the community, contributing to the development and implementation of patient-centered care plans.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license, or compact state license.
