We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. As an integral part of the organization, you will provide skilled nursing care to geriatric patients in the community, ensuring high-quality treatment and support.

Responsibilities

• Deliver skilled nursing care and prescribed treatment to geriatric patients.
• Implement patient-centered care plans in line with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED.
• Current DE LPN license or compact state license.
