Job Description
We are looking for a dedicated Home Care LPN to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. This position offers a competitive hourly salary of $22.00 - $28.00, flexible work hours, and comprehensive benefits including medical, dental, vision, 401(k), and tuition assistance.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
