Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As an integral part of the HomeHealth division, you will provide skilled nursing care to geriatric patients in the community and contribute to the development and implementation of patient-centered care plans.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
