Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a Home Care LPN, you will provide skilled nursing care and treatment to geriatric patients in the community. You will work closely with our supportive leadership team and leverage our state-of-the-art Electronic Health Record system for patient documentation.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in planning for patient discharge.
Requirements
- High School Diploma or GED required.
- Current DE LPN license, or compact state license.
