Pathways Vermont Homeless Outreach Specialist - $3000 Hiring Bonus Available*

Responsibilities:

• Engage in assertive outreach, service coordination, advocacy, and support for program participants.
• Build rapport with individuals in various settings.
• Facilitate telehealth or in-person meetings for mental health screening.
• Maintain accurate records in databases.
• Practice best support practices for persons experiencing homelessness.
• Facilitate client-driven referrals and participation in community activities.
• Other duties as assigned.

Qualifications:

• Driver's License and vehicle required.
• Flexible schedule and travel required.
• Lived experience with mental health challenges, substance use, or homelessness preferred.
• Bachelor's degree or equivalent experience.
• Patience, creativity, flexibility, compassion, and sensitivity required.
• Training in relevant areas preferred.
• Strong communication and computer skills.

Working Conditions:

• Physical requirements include occasional lifting, walking, and exposure to outside conditions.
• Compensation: $23-$25/hour with a $3000 Hiring Bonus after the introductory period.
