INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, and ensuring efficient use of health services.

Responsibilities

• Provide discharge planning coordination and intervention.
• Promote patient wellness and improved care outcomes.
• Efficiently utilize health services and minimize payment denials.

Requirements

• Bachelor's degree in nursing (BSN) from a CCNE accredited school.
• Basic professional knowledge in a specialized field.
• Experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager.
