INPATIENT CASE MANAGER (RN)

Job Description

We are currently looking for a dedicated Inpatient Case Manager (RN) to join the LifeBridge Health team. In this role, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions. The primary goal is to enhance patient wellness, improve care outcomes, ensure efficient health services utilization, and reduce payment denials for patients with complex health needs.

Responsibilities

• Coordinate discharge planning in conjunction with the clinical team and medical providers.
• Promote patient wellness and enhance care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Hold a Bachelor's degree with working knowledge within a specialized field.
• BSN required from CCNE accredited schools.
• Previous experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager at LifeBridge Health will be considered.
