We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position at Northwest Hospital in Randallstown, MD offers a sign-on bonus potential of $15,000. The role involves discharge planning coordination and intervention to promote patient wellness, improve care outcomes, and ensure efficient utilization of health services.

Responsibilities

• Collaborate with the clinical team and medical provider for effective care management.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• BSN required from CCNE accredited schools.
• Prior experience in an inpatient care management role preferred.
• Demonstrated ability to work effectively in a team environment.
