Job Description
ChristianaCare HomeHealth is seeking Licensed Practical Nurses (LPNs) to join our team in New Castle County, DE. As an LPN in our organization, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and participate in discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
