Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
Job Details
- Job Types: Per Diem, Part-time, Full-time.
- Pay: $22.00 - $28.00 per hour.
- Expected Hours: 20-40 per week.
