Duties and Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or equivalent required.
- Current Delaware LPN license or compact state license.
Job Details
- Job types: Per diem, Part-time, Full-time
- Pay: $22.00 - $28.00 per hour
- Expected hours: 20 – 40 per week
- Medical specialties: Geriatrics, Home Health, Pediatrics, Wound Care
- Standard shift: Day shift, Night shift
- Supplemental pay types: Differential pay
- Supplemental schedule: Holidays, Overtime
- Weekly schedule: 5x8, Monday to Friday, Rotating weekends
