Job Description
ChristianaCare HomeHealth is looking for dedicated Licensed Practical Nurses (LPNs) to provide skilled nursing care to geriatric patients in the community. This role involves implementing care plans, participating in discharge planning, and ensuring patient-centered care with available resources.
Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.

Work Schedule
