Job Description

Duties and Responsibilities
- Provide skilled nursing care to geriatric patients in the comfort of their homes.
- Develop and implement patient-centered care plans in coordination with the healthcare team.
- Participate in discharge planning to ensure continuity of care.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
