Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team. As an LPN at ChristianaCare, you will provide skilled nursing care to geriatric patients in the community. You will be responsible for implementing care plans, participating in discharge planning, and ensuring that treatments are patient-centered and in line with available resources.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license, or compact state license.

Pay
$22.00 - $28.00 per hour
