Job Description
ChristianaCare HomeHealth is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. As a key member, you will provide skilled nursing care to geriatric patients in the community, implementing care plans that are patient-centered and goal-oriented.
Duties and Responsibilities
- Give skilled nursing care and prescribed treatments to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
