Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, implementing care plans in a patient-centered manner. You will have the opportunity to support patients in New Castle County, DE, working either day or night shifts, with one weekend and two holidays required per year.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in accordance with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
