Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Execute goal-oriented care plans in alignment with patient needs and available resources.
- Contribute to patient discharge planning.
Requirements
- High School Diploma (HSD) or equivalent.
- Current DE LPN license or compact state license.
