Job Description
We are seeking dedicated Licensed Practical Nurses (LPNs) to join ChristianaCare's HomeHealth team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community while implementing patient-centered care plans. Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
