Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN at ChristianaCare, you will have the opportunity to provide skilled nursing care to geriatric patients in our community, ensuring they receive the highest level of care and support.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or equivalent required.
- Current DE LPN license or compact state license.
- Prior experience in home health is a plus.
