Job Description

Genesis Health is seeking a Licensed Practical Nurse MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a crucial role in our Resident Assessment Process by completing the Minimum Data Set and Care Area Assessments to ensure tailored patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in computer skills, including Windows applications.
