Position Highlights
- Direct and oversee the RAI process according to guidelines.
- Develop individualized Care Plans for residents.
- Provide training on MDS items to the interdisciplinary team.

Requirements
- Current LPN license.
- Prior experience in healthcare facilities.
- Knowledge of nursing practices and regulations.
- Computer proficiency, Windows applications.
- AccuMed experience preferred.
- Previous MDS Coordinator experience preferred.
- BLS/CPR certification required.
