Duties and Responsibilities:

• Provide skilled nursing care to geriatric patients in the community.
• Implement goal-oriented care plans.
• Participate in discharge planning.

Requirements:

• HSD or GED required.
• Current DE LPN license or compact state license.

Job Types: Per diem, Part-time, Full-time
Pay: $22.00 - $28.00 per hour
Expected Hours: 20 – 40 per week.
Medical Specialties: Geriatrics, Home Health, Pediatrics, Wound Care
Standard Shift: Day shift, Night shift
Supplemental Pay Types: Differential pay
Supplemental Schedule: Holidays, Overtime
Weekly Schedule: 5x8, Monday to Friday, Rotating weekends
