ChristianaCare HomeHealth is seeking experienced Licensed Practical Nurses (LPNs) for full-time, part-time, and per diem positions in New Castle County, DE. Join our team to provide skilled nursing care to geriatric patients in the community with a patient-centered approach. As part of our team, you will play a vital role in implementing care plans and ensuring the well-being of our patients.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
