As a Licensed Practical Nurse (LPN), you will have the opportunity to build trusted relationships with patients and play a key role in delivering high-quality care.

Job Summary

The Home Care Licensed Practical Nurse will be responsible for providing direct nursing care to acutely and chronically ill patients in their homes, collaborating with the healthcare team, and ensuring the highest quality of patient care. Under the supervision of a Registered Nurse or designated supervisor, the LPN will adhere to state regulations and agency policies to deliver exceptional care.

Responsibilities

• Deliver high-quality clinical services in alignment with the plan of care and state regulations.
• Document patient visits accurately and efficiently in the electronic medical record.
• Participate in care coordination processes to ensure effective communication and patient care.
• Maintain patient confidentiality and adhere to all agency policies and procedures.

Requirements

• Meet educational/experience requirements as per state licensure laws.
• Minimum of one year of acute care experience preferred; home care experience is a plus.
• Hold a current LPN licensure in the state of practice and possess a valid driver's license and vehicle insurance.
• Current CPR certification is required.
• Demonstrate strong patient management, documentation skills, and a commitment to organizational goals.
• Ability to work flexible hours to meet patient needs and adapt to changing organizational requirements.
