Job Summary

• Provide direct nursing care to acutely and chronically ill patients in their homes.
• Coordinate patient care activities to ensure the highest quality of patient care.
• Work under the supervision of a Registered Nurse or designated supervisor.

Responsibilities

• Deliver high-quality clinical services within the scope of practice and in accordance with the plan of care.
• Document patient visits thoroughly and adhere to all regulatory requirements.
• Participate in the agency's care coordination process to communicate effectively with the care team and other stakeholders.

Requirements

• Must meet educational and experience requirements as per state licensure laws.
• Minimum of one year of acute care experience preferred.
• Current LPN licensure, CPR certification, driver's license, and access to a vehicle/public transportation.
• Strong patient management, documentation, and interpersonal skills.
• Ability to work flexible hours and adapt to organizational needs.
