LPN MDS Coordinator

Job Description

We are looking for a dedicated LPN MDS Coordinator to join our team at Genesis HealthCare. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the nursing center's RAI process, adhering to guidelines for MDS completion and electronic transmission.
• Assist in creating personalized resident Care Plans, addressing resident needs and expected outcomes in compliance with Federal and State regulations.
• Provide training to new team members on clinical content and electronic MDS item completion.

Requirements

• Active LPN license and good standing.
• Previous experience in a healthcare setting is essential.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Knowledge of laws, regulations, and guidelines governing long-term care.
• Computer skills, including proficiency in Windows applications.
• AccuMed experience preferred.
• Prior MDS Coordinator experience is a plus.
• Current BLS/CPR certification required (excluding ALFs/ILFs).
